Service Checklist
Dear Customer:
If it is necessary to put the vehicle into our shop for diagnosis, so as to give this estimates, a charge will be imposed for its preparation. A minimum of one-half hour labor charge for any diagnosis. You are entitled to the return of old parts that were replaced in repair of your vehicles. Labor charges are computed flat rate unless otherwise specified. Unless otherwise specified, labor time billed is flat rate time estimated for each job in industry manuals and not actual time spent.

A charge of 10% of the total charge will be charged on all repair orders to defray the expense of shop materials. A charge of $89.95 per day will be made for storage commencing 24 hours after notification that repairs or service have been completed. We cannot provide inside storage and we are not responsible to damage caused by weather, or other circumstances beyond our control. We are sorry that cannot furnish a car for your use while repairs are made.

I hereby authorize the repair work hereinafter set forth to be done along with the necessary material and agree that you are not responsible for loss or damage to vehicle or articles left in vehicle in case of fire, theft or any other cause beyond your control or for any delays caused by unavailability of parts or delays in parts shipments by the supplier or transporter. I hereby grant you and/or your employees permission to operate the vehicle herein described on streets, highways or elsewhere for the purpose of testing and/or inspection. An express mechanic’s lien is hereby acknowledged on above vehicle to secure the amount of repairs thereto.
